  If any person is aggrieved by an order made by the District Forum in exercise of its powers conferred by Section 11 may prefer an appeal against such order to the State Commission within a period of thirty days from the date of the order on all working days between 10.00 A.M. to 4.00 P.M. at Plot No. 5-B,Sector 19-B,Madhya Marg,Chandigarh. The Memorandum of appeal in original should be duly signed by the Appellant (s) and must be supported by An affidavit along with three copies for the Commission and one set for each Respondent. Memorandum of appeal shall be in legible handwriting preferbly typed and shall set forth concisely under distinct heads of the grounds of appeal. Paper books should be duly paginated and indexed as per the seriatim given below:
1.  Index
2.  Name of Parties;
3.  Memo of Parties (with fresh complete addresses,telephone no., mobile no. and also email address.)
4.  Appeal with duly attested affidavit
5.  Stay Application, or any other application if any, with duly attested affidavit
6.  Application for condonation of delay with duly attested affidavit, if beyond limitation. (Within 30 days of      receipt of order)
7.  Certified copy of order of District Forum
8.  Statuary Deposits as per Section 15 of the Consumer Protection Act, 1986 in the form of Demand Draft in favour    of “Secretary, State Consumer Disputes Redressal Commission, U.T. Chandigarh”

Note: 1.The documents shall be filed in English language only or translated copy of any other language, duly attested.
